How it works
From a field library to a running workflow
Build fields, compose a form
Text, email, phone, number, date, dropdown, radio, checkboxes, consent and hidden fields, plus headings, paragraphs, images, dividers and page breaks. Create a field once, reuse it on every form, mark it required and add show-if conditions.
Put it on your storefront
As a theme block on any page, as a popup with its own trigger and frequency rules, or as a direct link you can share. All three use the same renderer and the same form.
Let Shopify Flow take over
Every submission fires the "Form submitted" trigger with all answers. Tag the customer, create a draft order, notify your team, send an email: any Shopify Flow action.

Forms built for automation
A form is not the end of the story: it is the start of a workflow. Workflow Forms collects the answers, keeps them safe and hands them to Shopify Flow.
Reusable field library
Twelve field types and six layout blocks. A field keeps its handle for life, so your workflows and conditions never break when you reorder or rename a form.
Show-if conditions
Show or hide fields based on earlier answers, cascading through the form. Conditions run in the browser and again on the server, so a hidden field is never submitted.
Multi-step forms
Insert page breaks to split a long form into steps with a progress indicator. A step that becomes empty through conditions is skipped automatically.
Theme block, popup or link
Place a form inline through the theme editor with styling per block, open it as a popup after a delay, a scroll depth, a number of page views or on exit intent, or share it as a page at a direct link.
Three Shopify Flow triggers
"Form submitted" with every answer, "Form submission rejected" when a visitor is refused, "Form submission status changed" when your team works the inbox.
Spam that never reaches you
Honeypot, signed render time, rate limits and blocked email domains. Spam is answered like a success and stored nowhere, so the sender learns nothing.
Submissions inbox and export
New, read and archived, bulk actions, a streamed CSV export that is safe to open in a spreadsheet. Answers are encrypted at rest and deleted on request.
Metaobject copy and API
Each submission can be copied into an app-owned metaobject for your theme or other apps, and the Developer API and MCP server manage forms, fields and submissions from your own code or an AI agent.

Every answer is available in Shopify Flow by its field handle, so a condition like "volume equals 500-1000" or an action that tags the customer with their interests is a few clicks.
